"Best of the Bay" BASS TOURNAMENT
Entry Form

• Please print and complete an entry form for your 4 person team (see below).
• Entry Fees for this event are $500.00 per FOUR PERSON TEAM. ($525.00 if done with credit card online).
• To be considered for one of the first 16 slots, entry forms along with full payment MUST be received at the SFVU office by NO LATER THAN June 5th, 2008.
• NO refunds will be issued after June 6th, 2008.
• NO entries will be accepted morning of the tournament.
• Entry Form MUST be COMPLETELY filled out and personally signed by ALL FOUR PARTNERS to constitute a legal entry.
• Social Security Numbers are REQUIRED as outlined by Federal 501 C-3 law.
• Entries accepted up to first 16 based on "first in" as of postmark, any additional entries starting with 17 will be held until the 32nd entry, when we will form a 2nd 16 team bracket. This insures a guarantee of equal brackets.
• This event is OPEN to all individuals EXCEPT fishing guides and individuals who make their primary living as professional fishermen, tournament or commercial.
• No membership is required.
• Anyone under 18 years of age may participate with written permission from parent or guardian.
• SFVU/FDU officials reserve the right to reject any entries we feel are not in the best interest of the fishing tournament event. Tournament director and staff are allowed to fish this event but with livewell check and final weigh-in of their fish verified by any other participant.

You may also pay by Credit Card through Paypal. A Paypal account is not required.
An additional $25.00 fee per Team will be added for online payments.

After making payment online,
please mail your completed entry form as described Above.
(Note: Entry Form must be signed by each person on the team.)
